About the Supply of Forensic Pathologists I have spent most of the last 20 years of my career in Forensic Pathology at the “Chief” level. As such I was responsible for staffing. I served at the “Chief” level in multiple states including Florida, Texas, and California. From my experiences I have a few observations: 1. The shortage hysteria is not new. During AP/CP residency and FP fellowship at the University of Colorado in 1989 I first became aware of the job issue. Pathology department chairpersons had published several articles in the CAP literature about the impending catastrophic shortage of pathologists including forensic pathologists. Their hysteria was based on the rising rates of unfilled residency positions. Many programs depend on resident labor. You know the rest. However, they tried to frame their argument as an aging population of pathologists who would retire without anyone to replace them. They predicted the end of pathology as we know it. Yet here we are 34 years later. Still screaming that the sky is falling. 2. There can not be a shortage in the face of falling wages. The laws of economics are not as immutable as those of thermodynamics but the one about supply and demand is pretty solid. If you look at inflation adjusted dollars forensic pathologist salaries have not made much progress over the past 30 years. I left Volusia County, FL in 2006 at a salary of $169,000.00. Adjusted to current dollars that would be $255,000.00. I left because they would not give me a raise after many years of service. Many of the jobs listed on the NAME job board are advertising less than $255,000.00. Another good measure is the amount county coroners will pay for an autopsy. In 1992, in the central valley of California, they were paying about $1200. Today over 30 years later in South Carolina they pay about $1400. If there is a shortage of forensic pathologists compensation (adjusted for inflation) would have gone up significantly. It is the law of supply and demand. 3. The people screaming “shortage” the loudest are those responsible for staffing their offices. Just like the pathology department chairpersons in the late 1980’s, the people who have to recruit and retain forensic pathologists are the ones complaining the most. That is because they don’t want to pay a decent wage. They want to get their autopsies done on the cheap. They want to put out a job notice and have an avalanche of applicants. Once hired they don’t want to worry that their pathologist will leave for something better. They want to work their forensic pathologist to exhaustion and not have to worry about the forensic pathologist having other opportunities. It is nothing more than getting the most autopsies for the fewest dollars. As a taxpayer, I’m not saying this is bad. I’m just saying that is what is truly generating all the noise about a shortage. Which brings me to. 4. Good jobs have no shortage of highly qualified applicants. When I was responsible for hiring, I had no shortage of applicants. If you offer a good compensation package and a comfortable working environment you will fill your position. If you have a good facility with plenty of staff support and stay below 250 they will never leave. If NAME wants to increase the number of Forensic Pathologists, focus on increasing salaries. For decades it has been said: “Forensic Pathology is the only medical specialty where you do a fellowship and get another board certification to cut your salary in half.” In closing, some perspective. In the United States I believe there are about 600 forensic pathologists actively (daily) doing autopsies. There are 650 Major League Baseball players. There are more than 1200 National Football League players.

Unfortunately mayo realigned their oncology dept and I was assigned team absent Dr Creagan. Fortunately I would still see him playing piano in the atriums at Mayo Rochester. Last summer he was playing the beautiful Bosendorfer in the Gonda Atrium. We had a wonderful conversation, he told me of his upcoming retirement. No one deserves retirement more than Ed Creagan, he spent countless hours helping those who often lose hope. Congrats and love from Mr R

I have glaucoma already. I got glaucoma at a young age of 9 and I've been on eye drops for 16 years. I have to do this for the rest of my life and I have to see a glaucoma doctor for the rest of my life as well. If I get off my eye drops I could go blind. I don't want this to happen so I I am dedicated to taking my eye drops every day three times. Glaucoma has definitely changed my life and it has taught me that not only old people can get this disease but young people to. If people ever came up to me and were shocked that I have something that is so common in an older person I would say yes that's true that older people can get glaucoma but young people can get it to how I know is I'm a prime example of getting it as a child.
